The device was not returned for evaluation; therefore, a device analysis could not be performed. The clinical/medical investigation concluded that as of the date this medical investigation the supporting clinical documentation has not been provided; therefore, no clinical root cause for the ¿broken spacer¿ can be confirmed.A review of the instructions for use documents for knee systems revealed in warnings and precautions that the implant can break or become damaged as a result of strenuous activity or trauma.Device specific identifiers were not provided. Therefore, an evaluation of the manufacturing records, complaint history review, risk management file, prior actions and product prints could not be performed. At this time, we have no evidence to conclude that the product failed to meet any specifications at the time of manufacture.Factors that could contribute to the reported event include patient anatomy, abnormal loading of limb, excessive forces and/or traumatic injury.Based on this investigation, the need for corrective action is not indicated.Should the device or additional information be received, the complaint will be reopened. No further investigation is warranted for this complaint; however, we will continue to monitor for future complaints and investigate as necessary. We consider this investigation closed.

H3, h6: the device was not returned for evaluation. However, the photograph was reviewed, and revealed that the device is fractured into pieces. The clinical/medical investigation concluded that although a definitive clinical root cause of the reported catastrophic polyethylene wear cannot be concluded based on the information provided, it is likely the patient¿s index body mass and increased joint laxity after 7 years in-vivo contributed the reported event as intraoperative findings are consistent with impingement and subsequent deformation of the articulating insert posteriorly, which can also be seen with subluxation and/or 3rd body wear. The repeated physical therapy sessions after the symptoms began could have accelerated the wear.It is unknown if the posterior cruciate ligament remained intact or if a deficiency/injury could have contributed to the reported increased instability, pain ¿behind knee¿, and ¿locking-type episodes¿ as trauma was not reported.Additionally, the approximate year-delay to diagnosis and treatment after follow-up with 4 different physicians following the initial complaint of the medial-sided right knee pain certainly would have contributed to the severity of the patient symptoms and component deformation.A component malperformance cannot be confirmed based on the documentation provided.A review of the instructions for use documents for knee systems revealed in warnings and precautions that the implant can break or become damaged as a result of strenuous activity or trauma.Device specific identifiers were not provided. Therefore, an evaluation of the manufacturing records, complaint history review, risk management file, prior actions and product prints could not be performed. At this time, we have no evidence to conclude that the product failed to meet any specifications at the time of manufacture.Factors that could contribute to the reported event include patient anatomy, abnormal loading of limb, excessive forces and/or injury.Based on this investigation, the need for corrective action is not indicated.Should the device or additional information be received, the complaint will be reopened. No further investigation is warranted for this complaint; however, we will continue to monitor for future complaints and investigate as necessary. We consider this investigation closed.

H3,h6: the device was not returned for evaluation. However, the photograph was reviewed and revealed that the device is fractured into pieces. The clinical/medical investigation concluded that, the provided photocopied x-ray appears to show a lack of space medially between the femoral and tibial components which is consistent with a lack of an intact articulating insert.Although a definitive clinical root cause of the reported catastrophic polyethylene wear cannot be concluded based on the information provided, it is likely the patient¿s index body mass and increased joint laxity after 7 years in-vivo contributed the reported event as intraoperative findings are consistent with impingement and subsequent deformation of the articulating insert posteriorly, which can also be seen with subluxation and/or 3rd body wear. The repeated physical therapy sessions after the symptoms began could have accelerated the wear.It is unknown if the posterior cruciate ligament remained intact or if a deficiency/injury could have contributed to the reported increased instability, pain ¿behind knee¿, and ¿locking-type episodes¿ as trauma was not reported.Additionally, the approximate year-delay to diagnosis and treatment after follow-up with 4 different physicians following the initial complaint of the medial-sided right knee pain certainly would have contributed to the severity of the patient symptoms and component deformation.A component malperformance cannot be confirmed based on the documentation provided.A review of the production order did not reveal a manufacturing abnormality that could have caused or contributed to the reported incident.A review of complaint history for the part number over the past 12 months and for the batch number based on historical data of the device did not reveal similar events for the listed device.A review of the instructions for use documents for knee systems revealed in warnings and precautions that the implant can break or become damaged as a result of strenuous activity or trauma.A review of the risk management file revealed this failure mode was previously identified. The anticipated risk level is still adequate.A historical review concluded that there are no prior actions related to this product and event.According to the inspection drawing, final inspection includes a visual verification that parts are free of damaged areas, burrs, steps, inclusions, porosities, sharp edges and cracks. At this time, we have no evidence to conclude that the product failed to meet any specifications at the time of manufacture.Based on this investigation, the need for corrective action is not indicated.Should the device or additional information be received, the complaint will be reopened. No further investigation is warranted for this complaint; however, we will continue to monitor for future complaints and investigate as necessary. We consider this investigation closed.
