It was reported the physician was implanting a 37mm gore® cardioform asd occluder to close an atrial septal defect. The device could not fully capture the superior rim, so it was decided to remove the device. The angle made it difficult to recapture the device, and it caught on the tip of the delivery system, breaking the retrieval cord. The device embolized into the right ventricle. The device was then snared out of the pulmonary artery.A new 44mm gore® cardioform asd occluder was implanted with no issues. The patient was doing well following the procedure.
