Correction: d4 lot unknown.A review of the device history record could not be performed because a valid lot number was not provided; therefore, the unique device identifier¿production identifier was unavailable.Sample evaluation was not performed because the device was not returned for analysis.Based on review of the available tracing information, the reporting avanos representative indicated that there was a clear deviation from the vertical midline consistent with lung placement, and the patient subsequently experienced a pneumothorax. The cortrak 2 enteral access system operator's manual instructs users to stop insertion and immediately withdraw the tube if the tube deviates from the vertical midline, respiratory irritation or distress occurs, or resistance is encountered. The operator's manual also warns that the cortrak 2 enteral access system does not prevent patient injury resulting from misplacement of a feeding tube into the trachea or lungs and does not replace qualified clinical judgment, supervision, or vigilant patient assessment during feeding tube placement. The root cause was incorrect use.All information reasonably known as of 23 jun 2026 has been included in this health authority report.Should additional information be obtained, a follow-up health authority report will be provided. The information provided by avanos medical inc.Represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant/reporter was unable or unwilling to provide any further patient, product, or procedural details to avanos medical inc.Avanos medical inc.Has no independent knowledge of the reported event but is relaying the information provided by the user facility where the incident occurred. This product incident is documented in the avanos medical complaint database and identified as complaint (b)(4).This information is submitted pursuant to 21cfr803, in compliance with the medical device reporting requirement and should not be considered to be an admission that an avanos medical inc.Product is defective or caused serious injury.
