Additional Manufacturer Narrative
Product complaint # (b)(4).Investigation summary: according to the information received, "because the liner is damaged or cracked, replace the attune-rp-liner".The product was not returned to j&j medtech orthopaedics. However, photos were provided for review.(b)(4).The photo investigation revealed that the attune ps rp insrt sz4 6mm displays what appears to be deep scratches and marks on the overall surface.Additionally, the central post appears to be fractured and/or cutted. However, due to the angle and quality of the provided photo, the fractured surface was not able to be analyzed, and no cracks were identified. With the available information, it is not possible to determine the cause of the observed condition of the device. However, it is suspected that the scratched were due to attempts of implantation and contact with surgical instruments. As per attune¿ knee system intuition ¿ surgical technique rev.K, on page 78, the next steps need to be followed for a proper implantation of the attune ps rp insrt "for a rotating platform implant, place the rp trial post into the implanted base component. Then place the insert trial over the post and perform the trial reduction. For rotating platform components, verify rotational stability with pcl tension.Remove loose fragments or particulates from the final tibial base. For rotating platform tibial components, insert the final tibial insert. As the device was not returned, an as-received condition could not be assessed, and a dimensional inspection and document/specification review were not completed. The overall complaint was confirmed as the observed condition of the attune ps rp insrt sz4 6mm would have contributed to the complained device issue.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of j&j medtech orthopaedics quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot: the product investigation found no evidence suspecting an error in the manufacturing or material that would be a contributing factor in the reported allegation(s).A manufacturing records evaluation (mre) was not performed.H11 additional narrative: corrected: h6 (pe code was updated from implant fracture post-op: polyethylene to implant fracture intra-op: polyethylene.).
