H3 device evaluation summary: medtronic conducted an investigation based upon all information received. It was reported that this 980 ventilator did not start, emitted smoke from the "cage computer" along with the graphical user interface (gui) monitor, and smelled like burnt plastic. The device was available for evaluation. The service personnel (sp) inspected the ventilator and found that upon start up the ventilator alarmed, and smoke was observed from both the expiratory side of the breath delivery unit and from the graphical user interface (gui) with a smell of burnt plastic.Upon further inspection, it was observed that the breath delivery (bd) power controller and power distribution printed circuit board assemblies (pcba) and the user interface (ui) pcba had burnt components. The cause of the reported issue was isolated in the field to a potential fault in the bd power controller pcba and/or bd power distribution pcba and/or the ui pcba. The event is included in trending and monitoring.Medtronic submits this report to comply with fda regulations 21 cfr parts 4 and 803.Medtronic has made reasonable efforts to provide as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.Medtronic will submit a supplemental report if additional relevant information becomes known.
