As reported by the field, this was an endovascular embolization of major aortopulmonary collateral arteries (mapca).A 5fr diagnostic catheter and prowler select plus were advanced to internal thoracic artery, and the coil embolization was initiated. After 5 j&j coils were implanted to internal thoracic artery, the deltafill18 4mm x 15cm coil (dlf180415, 31178117) was used as the sixth coil.During insertion, the microcatheter (mc) jumped and deviated from the target site. When the coil was pushed and pulled several times, the coil got early detached inside of the microcatheter. The coil was retrieved from the patient¿s body with the microcatheter (mc). After that, the embolization to internal thoracic artery was completed, and another 5 j&j coils were implanted to another branch. The angiography was conducted to confirm, the procedure was successfully completed. There was no impact to the patient.A continuous flush was done. The lesion was right internal thoracic artery.

Product complaint # (b)(4) updated sections on this med watch report: b4, g3, g6, h2, and h11.Complaint conclusion: as reported by the field, this was an endovascular embolization of major aortopulmonary collateral arteries (mapca).A 5fr diagnostic catheter and prowler select plus were advanced to internal thoracic artery, and the coil embolization was initiated. No additional information is available. The device was discarded; therefore, no further investigation can be performed.A manufacturing record evaluation was performed for the finished device 31178117 number, and no non-conformances related to the malfunction were identified. With the information available and without the product available for analysis, the reported customer complaint could not be confirmed.Based on the manufacturing record evaluation, there is no indication that the event is related to the device manufacturing process. The exact cause of the event could not be determined; however, there are circumstances of the procedure that may have contributed to the reported failure.Premature detachment is a known potential procedural complication associated with the deltafill18 coil.Excessive force applied to a coil through repeated coil deployments or manipulation can increase the possibility of unintended detachment. As part of the post market surveillance program, information from this complaint is trended to identify statistical signals for consideration of further correction action.Since there was no evidence to suggest the event was related to a manufacturing or design issue, no corrective actions will be taken at this time.A supplemental report will be submitted if new facts arise which materially alter information submitted in a previous mdr report.
