H11: section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.The complaint of a leak was confirmed, but the root cause could not be determined. The product returned for evaluation was one 20g powerloc infusion set.An injection cap was returned attached to the luer of the infusion set.The returned unit was leak tested by flushing the infusion set with water using a 12ml luer lok syringe.During testing, a leak was observed at the interface between the returned injection cap and the infusion set luer. The unit was tested again twice, once with a representative injection cap and once with a direct connection to the luer lok syringe. In both instances, the unit was found to leak at the luer, indicating that the leak was being caused by the infusion set and not by any device being attached to it.Microscopic inspection of the infusion set luer found that a portion of the inner luer wall was damaged. The damage extended to the proximal end of the luer.Around the edges of the damaged segment, the luer wall material was frayed. It is likely that the observed damage prevented an adequate fit to occur between the luer and the injection cap, thus, creating a path for liquid to flow through. However, based on the available evidence, it could not be conclusively determined when or how the damage occurred. Therefore, the root cause remains unknown. This complaint will be recorded for future trending and monitoring purposes.
