Event or Problem Description
It was reported that during a procedure involving an onyx frontier coronary drug eluting stent, stent dislodgement occurred when pulling back from the distal to mid part of the lesion. The lesion was severely tortuosity, severely calcification with a 90% stenosis and located in the mid-distal right coronary artery (rca). The lesion was pre-dilated.The device did not pass through a previouslydeployed stent.Resistance was not encountered when advancing the device.Excessive force was not used. The stent was successfully removed using balloon inflation. The patient is alive with no further injury.

Additional Manufacturer Narrative
Additional information: it was detailed that the stent was advanced past the lesion and then was pulled back for proper overlap to the distal stent that it was previously placed with no issues.Resistance was experienced in pulling the stent back which was believed to have the stent dislodged.Excessive force was used. The stent was removed during the procedure.Medtronic submits this report to comply with fda regulations 21 cfr parts 4 and 803.Medtronic has made reasonable efforts to provide as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.Medtronic will submit a supplemental report if additional relevant information becomes known.
