It was reported by the customer, picc insertion nurse contacted informing that 24 hours after catheter insertion, image showed that part of the guide wire was in the pulmonary artery.Patient had to undergo hemodynamic procedure to remove foreign body.Additional information received on october 30, 2024: it was reported the fragment removed from the pediatric patient¿s pulmonary artery.Physical harm due to an invasive medical procedure.Impact on health due to the need for intervention and diagnosis of thrombosis. In addition to extending the period of hospitalization. After inserting the picc, the patient presented symptoms of dyspnea. An x-ray was requested and a foreign body was identified in the chest region. The catheter stylet broke and a fragment lodged in the pulmonary artery, requiring catheterization intervention in hemodynamics to remove it.A few hours after the procedure, a thrombus was diagnosed in the pulmonary artery, requiring the patient to be treated with anticoagulation. The patient remains hospitalized, under observation, using the inserted catheter with continuous medication to treat the thrombus. The picc radiology was inserted on (b)(6) 2024, performed by a nurse in training and the nurse (multiplier).According to the nurse, it was not a simple insertion, and there were some difficulties during the procedure. The first difficulty was related to the needle guide, since a 21g beveler was being used, while the power picc radiology needle is 20g, which made handling difficult due to the looseness in the fixing of the components.Another problem reported was the size of the hydrophilic stylet, which was 70 cm, while the catheter was 55 cm.During the procedure, the internal stylet of the catheter was damaged due to the difficulty in inserting it, bending the tip.Because of this, the nurse cut a few centimeters and resumed inserting the catheter, completing the insertion without complications. The nurse did not report noticing any rupture of the material during insertion. An x-ray was performed, which identified a different image.Initially, no action was taken. After a few hours, the patient developed dyspnea, requiring further tests, which detected a foreign body in the pulmonary artery, requiring an invasive medical procedure in hemodynamics to remove it.The catheterization lasted approximately two hours. The lodgement of the fragment caused a thrombus in the pulmonary artery, requiring intravenous anticoagulation. The patient remains hospitalized, under observation, in stable condition.
