It was reported that the subject device displayed an e126 error during set up.There were no reports of patient involvement.

This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.Updated fields: d8, h3, h4, and h6.The device was returned to olympus for inspection, and the reportable malfunction was confirmed.In addition, a b30 scope communication error was found during the device evaluation.A definitive root cause could not be identified.Based on the results of the investigation, it is likely the following led to the malfunction: damage or deterioration of parts, environment problem like as dust/dirt/humidity, optical problem, overheating problem, and electrical problems like as signal loss or open circuit.Olympus will continue to monitor field performance for this device.
