Additional Manufacturer Narrative
Product complaint # (b)(4).Updated sections on this med watch report: b4, g3, g6, h2, h3, h6, and h11.Complaint conclusion: as reported by the field, there was a hub leakage on an envoy 5f, mpd 90cm catheter (55625890, 31189193) when it was opened. No additional information is available.Since no device has been received for analysis, no product investigation can be performed and the reported failure could not be evaluated.A manufacturing record evaluation was performed for the finished device 31189193 number, and no non-conformances related to the malfunction were identified. With the information available and without the product available for analysis, the reported customer complaint could not be confirmed.Based on the manufacturing record evaluation, there is no indication that the event is related to the device manufacturing process.Multiple attempts to obtain additional information were unsuccessful. If information is provided at a later date, the file will be reopened and processed accordingly. As part of the post market surveillance program, information from this complaint is trended to identify statistical signals for consideration of further correction action.Since there was no evidence to suggest the event was related to a manufacturing or design issue, no corrective actions will be taken at this time.A supplemental report will be submitted if new facts arise which materially alter information submitted in a previous mdr report.

Additional Manufacturer Narrative
Manufacturer ref# (b)(4).Updated sections on this medwatch: b4, g3, g6, h2, h3, h6 and h11.The device was returned to j&j medtech for further evaluation.A non-sterile envoy 5f, mpd 90cm was received contained in the decontamination pouch.Upon receiving the device, visual inspection was performed, and multiple kinks were found along the entire length of the catheter. No other damages were noted.Microscopic inspection on the hub did not reveal any damage. The catheter was connected to a lab sample syringe, and the catheter was flushed. No leakage was noted from any part of the hub nor the shaft of the catheter.A manufacturing record evaluation was performed, and no internal actions related to the reported complaint condition were identified. The customer complaint cannot be confirmed with the evidence obtained from the analysis. There is no indication that the issue reported in the complaint is related to a manufacturing issue. The kinks were not originally reported, and the exact time of occurrence cannot be determined. It is suggested that these damages could be the result of the handling post-procedure of the device; therefore, these are not considered related to the issue reported. As part of the j&j medtech quality process, all devices are manufactured, inspected, and released to approved specifications.Since there was no evidence to suggest the event was related to a manufacturing or design issue, no internal action activity is required at this time.It should be noted that product failure is multifactorial. The instructions for use contain the following precaution: ¿ do not use a catheter that has been damaged in any way.If damage is detected, replace it with another envoy guiding catheter that is not damaged.Additional complaint information monitoring for potential safety signals is conducted through complaint trending as part of post-market surveillance.
