Complaint allegation is confirmed. One unpackaged, ar-10010, probe - hook, batch 12555658, was received for investigation.Visual evaluation noted that the probe hook was bent.Additionally, the handle of the device was faded and had nicks throughout it.Functional testing is not possible due to the damage of the device. The most likely cause for the reported failure can be attributed to misaligned insertion; excessive force while prying/leveraging the device during insertion.
