Event or Problem Description
It was reported that the subject device had an abnormal image that was sometimes disturbed by streaks.The event occurred during reprocessing.There was no patient involvement.

This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.Updated fields: d8, d9, e3, h2, h3, h6, h11.The device was returned to olympus for inspection and the reported failure was confirmed.In addition, the following reportable malfunction was identified during the device evaluation: malfunction of the video component, adapter and ccd cannot be split, interference stripes on image.A definitive root cause could not be identified.Based on the results of the investigation, it is likely the following led to the malfunction: regarding the customer's allegation, the color of the image was abnormal and disturbed by streaks due to a component failure, which is the same reason for the video component malfunction.The adapter and the charged coupled device cannot be split due to wear and tear problem.Should additional relevant information become available, a supplemental report will be submitted.Olympus will continue to monitor field performance for this device.
