Event or Problem Description
It was reported that a patient underwent a primary breast augmentation surgery with implantation of a 295cc mentor memorygel breast implant prosthesis.Post-operatively, the patient was diagnosed by a medical professional with bilateral mild breast ptosis in which the right breast nipple areolar complex was low and the left was even lower. As a result, the patient underwent bilateral mastopexy and bilateral breast implant removal without replacements on (b)(6) 2024.However, during the surgery, the right breast implant was found to be ruptured.A 60-miunute procedural delay was reported without any patient consequences or additional procedures. This report is for the right breast prosthesis.

Additional Manufacturer Narrative
On december 13, 2024, the mentor analysis lab received the suspect medical device for evaluation. On december 18, 2024, mentor completed an evaluation on the returned device.Mentor conducted a visual inspection, microscopic examination, and thickness measurement of the device.Visual analysis of the returned sample revealed that breast implant was found to be ruptured and received in three (3) parts.Microscopic examination was performed, and the cause of the tear could not be identified. Therefore a thickness measurement was conducted on the shell at the tear, and the thickness was within manufacturing specifications.Although no conclusion could be reached on the cause of the reported event, the instructions for use contain the following caution: rupture can occur at any time after implantation but is more likely to occur the longer the implant is implanted. The following may cause the implant to rupture: stressing the implant during implantation and weakening it; folding or wrinkling of the implant shell; excessive force to the chest (e.G.During closed capsulotomy); trauma; compression during mammographic imaging; and severe capsular contracture.Breast implants may also simply wear out over time. Some breast ptosis is a normal component of the mature breast.Ptosis becomes undesirable when the breast parenchyma predominates below the areola, droops considerably below the inframammary fold and the nipple points downward.Augmentation alone, without consideration of the ptosis can produce a less than desirable cosmetic result known as a "rock in sock" deformity or an increased ptotic appearance.Ptosis is a known complication associated with these devices and is referenced in our current product insert data sheet.Manufacturer¿s reference number: (b)(4).
