This report is being submitted as follow up no.1 to provide the device return date in section d9, update section h3, and to provide the completed investigation results. One r2p sheath was returned for assessment. The sample was subject to visual analysis. The sheath hub is separated and uncoiling. The sheath is kinked 2.3-3.7cm from the sheath tip.The sheath hub was placed under x-ray to check for the swage band. The swage band was identified under x-ray imaging. The band is shifted from its normal position in the sheath hub.The sheath hub was cut open to identify the layers of the r2p sheath.Upon opening the sheath hub, the inner layer, outer layer, strain relief and metal coil were identified. The complaint can be confirmed for sheath hub separation. The exact root cause cannot be determined. The likely root cause is the user pulled on the hub, leading to the separation. The operations quality engineer was notified about this issue and determined it is likely not a manufacturing issue as all layers of the sheath were identified and the swage band is present in the sheath hub.The device history record was reviewed to ensure that each manufacturing and inspection operation was performed. The review determined the process was performed and completed in accordance with terumo medical corporation specifications and procedures and the device was released in a conforming state.Currently, no action is recommended since the risk evaluation is within the predetermined limits in the hazard based risk table (hbrt).
