Manufacturer's ref.No: (b)(4).The purpose of this mdr submission is to report that multiple attempts to obtain product for analysis were unsuccessful. If product is returned or information provided at a later date, the file will be updated accordingly.Product analysis cannot be conducted as the product was not returned for analysis. No determination of causes and possible contributing factors could be made.As such, the investigation will be closed.A review of manufacturing documentation associated with this lot (31272794) presented no issues during the manufacturing or inspection processes related to the reported complaint. There were no nonconformances related to device manufacture or inspection. With the information available and without the complaint product available to be returned for analysis, the reported issue documented in the complaint cannot be confirmed through functional evaluation and analysis.Based on the manufacturing documentation review, there is no indication that the event is related to the device manufacturing process.Assignment of root cause for the event remains speculative and inconclusive, based on the limited information provided and without the return of the complaint sample; however, it is possible that clinical and procedural factors, including device manipulation/interaction may have contributed to the reported failure. As part of the post market surveillance program, information from this complaint is trended for statistical signals and corrective/preventive action may be triggered at a later time.Since there was no evidence to suggest the event was related to a manufacturing or design issue, no corrective actions will be taken at this time. The manufacturer will submit a supplemental report if new facts arise which materially alter information submitted in a previous mdr report.Additional information will be submitted within 30 days of receipt.

Manufacturer¿s ref.No: (b)(4).The purpose of this mdr submission is to report the investigational finding of the returned device. This mdr submission also includes a correction to the primary udi number. The complaint product was returned and received for evaluation and analysis. The investigation is documented below.Investigation summary: a non-sterile envoy da, 6f, 95cm, mpd was received contained in the decontamination pouch.Visual inspection was performed. No appearance of damage was found.Functional test was attempted, however, when the device was connected to the rhv it was noted that the hub was fractured.A manufacturing investigation was required. The catheter was connected to a y-connector, and the hub was found cracked at the fusion joint.Water was leaking from the cracked condition.Manufacturing investigation: the returned device was analyzed by the manufacturing team and after performed the analytical analysis via fourier transform infrared spectroscopy (ftir) and thermal stability-thermal gravimetric analysis (tga) test.Internal actions were taken as part of the non-conformance and were implemented on 10-jan-2025, after the manufacturing and release of the affected lot 31272794 (manufactured / released on 27-feb-2024 and reported as the complaint on 28-feb-2025).According to process failure mode and analysis (pmfea), there is one row that indicates the failure mode reported in the complaint: leakage from the hub, classified as low risk (bar), caused by an issue with the incorrect iso luer taper.During the investigation, the molding process was reviewed, and no correlation was found between the failure mode and the mold, molding machine, or defined parameters. As a result, based on the risk analysis, no further actions are required.A review of manufacturing documentation associated with this lot (31272794) presented no issues during the manufacturing or inspection processes related to the reported complaint. There were no nonconformances related to device manufacture or inspection. As part of johnson & johnson medtech quality process, all devices are manufactured, inspected, and released to approved specifications. Conclusion: based on the manufacturing review, there is no evidence to suggest that the root cause of the reported issue documented in the complaints is related to the manufacturing process. Therefore, no further actions are required. This issue is being addressed through johnson & johnson medtech quality system. As part of the post market surveillance program, information from this complaint is trended for statistical signals and corrective / preventive action may be triggered at a later time.Since there was no evidence to suggest the event was related to a manufacturing or design issue, no corrective actions will be taken at this time.Corrected section: d.4: primary udi number.Updated sections: b.4, d.4, g.3, g.6.H.2, h.3, h.6, and h.11.The manufacturer will submit a supplemental report if new facts arise which materially alter information submitted in a previous mdr report.Additional information will be submitted within 30 days of receipt.
