Additional information was received that the patient was being considered for a valve-in-valve procedure. It was further reported that a thrombus was not confirmed, but it was suspected based on an early elevated gradient that had not resolved on anticoagulation therapy. After the valve was initially implanted, the patient went home on blood thinner medication for 3 months. The patient¿s mean gradient at that time was 12 millimeters of mercury (mm hg).The patient returned 1 year later with a mean gradient of 27 mmhg and their medication was changed 3 times to different anticoagulation medications. The patients most recent gradient was 49 mmhg.
