H6: evaluation codes updated.Device evaluation: one sample was received.A visual inspection found that the yellow rotating collar was detached from the filter.Customer reported complaint was confirmed. The probable cause of the issue is that the connection was overtightened causing the collar to dislodge from the filter body.A review of the device history records shows there were no observations recorded during manufacture to suggest an issue of this nature would occur with this lot of product.
