Product complaint #(b)(4).Investigation summary: the following device was received for analysis: 952211500, lot no -nt0109.There was no product malfunction or patient harm reported with the returned device. The product was returned to j&j medtech orthopaedics for evaluation.Visual inspection revealed impaction marks on the overall body of the corail broach handle, including the lever of device.A functional testing was performed with a sample mating broach (201001060) revealed that the lever is loose, the broach handle was able to hold the broach as design intended. However, it was noted that the lever does not presents a tight fit.Based on observations on the device, the potential cause can be attributed to an unintended use since impaction forces applied to device on not intended areas could result on affecting mechanism which is not designed to absorb them.Properly handling and attention to the approved use of the device diminishes the risk of failure.A dimensional inspection was not performed since it was not applicable to the complaint condition. The overall complaint was confirmed as the observed condition of the corail broach handle would have contributed to a device issue.Based on the investigation findings, the potential cause is traced to unintended use error, and it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of j&j medtech orthopaedics quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot: a device history record (dhr) review or manufacturing records evaluation (mre) was not possible because the date code (nt0109) provided is not a valid finished goods lot# h11 additional narrative: corrected: h3.
