Product complaint # (b)(4).Investigation summary the cup impactor tip broke off the cup while getting impacted.Surgeons had to use a needle driver to remove the threaded tip from the impactor from the implant before impacting liner. The impactor did break into 2 pieces and all were removed. This incident did cause a 5 minute delay to remove the threaded tip and open a second impactor. The product was returned to j&j medtech orthopaedics for evaluation.Visual analysis of the returned device found that the weld on the threaded tip was broken. The broken fragment was returned for evaluation.Additionally the tip has signs of deformation. The observed conditions are consistent with the exposure to unintended forces like striking the device in areas that are not intended to be impacted, or impacting the device before the tip is fully seated into the cup, this threaded tip may separate from the shaft as a consequence of these unintended forces that likely caused the weld fracture.A functional test was unable to be performed since it was not applicable to the complaint condition.A dimensional inspection was not performed as it is not applicable to the complaint condition. The overall complaint was confirmed as the observed condition of the cas straight cup inserter would have contributed to the complained issue.Based on the investigation findings, the potential cause is traced to unintended use error, and it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of j&j medtech orthopaedics quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot a device history record (dhr) review or manufacturing records evaluation (mre) was not possible because the date code (j1107) provided is not a valid finished goods lot#.
