Event or Problem Description
It was reported that, during an internal fixation surgery, a extended ha pin 4.5mm 30mm x 150mm was inserted and checked on xray.It was decided the pin was inserted too far so the surgeon attempted to back it out slightly. The pin sheared off.A non-significant delay was reported. The patient's health status or how the procedure was finished is still unknown.

Additional Manufacturer Narrative
Updated results of investigation: the device was not returned for evaluation; therefore, a device analysis could not be performed.Based on the limited information provided, the definitive root cause of the reported extended ha pin breakage could not be determined.Although a procedure vs user error could not be ruled out.The extended ha pin is implantable; therefore, a foreign body reaction is not anticipated, and micro/migration of the retained pin is unlikely.According to the report, the procedure was completed with the same device with a non-significant surgical delay. The impact to the patient was the retained broken pin.Further impact can only be presumed but regularly scheduled follow-up/monitoring of the patient¿s status would be anticipated.A review of the production order did not reveal a manufacturing abnormality that could have caused or contributed to the reported incident.A review of complaint history for the part number over the past 12 months and for the batch number based on historical data of the device did not reveal similar events for the listed device.A review of the risk management file revealed this failure mode was previously identified. The anticipated risk level is still adequate.A historical review concluded that there are no prior actions related to this product and event.Product drawings and product inspection drawings were reviewed. No indication was detected which could have contributed to the reported failure mode.At this time, we have no evidence to conclude that the product failed to meet any specifications at the time of manufacture.Factors that could contribute to the reported event include size selected, surgical technique and/or patient anatomy.Based on this investigation, the need for corrective action is not indicated.Should the device or additional information be received, the complaint will be reopened. No further investigation is warranted for this complaint; however, we will continue to monitor for future complaints and investigate as necessary. We consider this investigation closed.
