It was reported that during diagnostic procedure the subject device had did not display image sometimes.The issue had occurred during sedation.There were no reports of patient harm.

This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.Updated fields: d4, d8, h2, h3, h4, h6 and h11.The device was returned to olympus for inspection and the reported failure was confirmed.In addition, the following reportable malfunctions were identified during the device evaluation: corrosion of video connector contact point, foreign material on the video connector contact point.A root cause could not be identified.Based on the results of the investigation, the cause of the no image was likely due to the failure of the converter.Should additional relevant information become available, a supplemental report will be submitted.Olympus will continue to monitor field performance for this device.
