H10: h3, h6: the reported device was not returned to the designated complaint unit for independent evaluation. However, a clinical review states that the clinical root cause was the result of a user technique vs procedural variance based on the report that the surgeon drilled off axis into what was noted as poor bone quality. The microraptor knotless anchor is implantable, so biocompatibility is not an issue.Local irritation/discomfort should not be ruled out.Micro-motion or movement cannot be predicted. There is potential risk for tissue inflammation and/or pain.A review of device records showed there were no indications to suggest that the product did not meet manufacturing specifications upon release for distribution.A complaint history review found similar reported events.A risk management review found that the reported failure and/or harm was documented appropriately, and there were no indications to suggest the anticipated risk is not adequate.A review of the composition device component found that the storage requirements for the material are specified, and the material must comply with provided percentage composition specifications as measured by ash test. The instructions for use was reviewed and found to include conditions of off label use and technique specifics, as well as precautions and warnings related to the use of the device. There is a definitive clinical root cause.Factors that could have contributed to the reported event include procedural variance, or deviations from established protocols, increasing risks and leading to unintended consequences.Based on this investigation, the need for corrective action is not indicated.
