Additional Manufacturer Narrative
Manufacturer¿s ref.(b)(4).The purpose of this mdr submission is to report the investigational finding of the returned device. The complaint product was returned and received for evaluation and analysis. The investigation is documented below.Investigation summary: a non-sterile 3mm x 8cm galaxy g3 xsft was received contained in the decontamination pouch.Visual inspection was performed. As noted in the photo that was included with the complaint, the core wire is kinked in a 90-degree angle.Microscopic inspection was performed.Under magnification, the embolic coil was observed to be severely stretched and still attached to the resistance heating (rh) coil. The issue documented in the complaint that the core wire was kinked during the re-sheathing was confirmed based on the appearance of the returned device. It is possible that excessive manipulation may have resulted in core wire kinking. With the limited information available, the root cause cannot be determined. There is no evidence to suggest that the issue reported is a result of a defect inherently related to the device. The coil stretching was not originally documented in the complaint, and its exact time of occurrence cannot be determined since no further information was gathered from the customer. It is possible that the coil became kinked during the unit withdrawal due to rotating hemostasis valve (rhv) overtightening or during the post-operative handling of the device. This condition is not considered related to the issue as reported.A review of manufacturing documentation associated with this lot (30860247) presented no issues during the manufacturing or inspection processes related to the reported complaint. There were no internal actions related to device manufacture or inspection. As part of johnson & johnson medtech quality process, all devices are manufactured, inspected, and released to approved specifications. It should be noted that product failure is multifactorial. The instructions for use (ifu) contains the following caution: - do not fasten the rhv valve too tightly around the introducer sheath since excessive pressure may cause damage to the introducer sheath and/or the microcoil as it is advanced into the infusion microcatheter.Additionally, if the introducer tip and microcatheter hub are misaligned, damage may occur to the microcoil as it passes through this transition.Based on the manufacturing documentation review, there is no indication that the event is related to the device manufacturing process. As part of the post market surveillance program, information from this complaint is trended for statistical signals and corrective / preventive action may be triggered at a later time.Since there was no evidence to suggest the event was related to a manufacturing or design issue, no corrective actions will be taken at this time.Updated sections. The manufacturer will submit a supplemental report if new facts arise which materially alter information submitted in a previous mdr report.Additional information will be submitted within 30 days of receipt.
