It was reported the gastrointestinal videoscope plastic distal end cover was burned.The issue occurred during service / maintenance.There were no reports of patient involvement.

This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation and additional information.The device was not returned for evaluation and the customers complaint was confirmed.Based on the results of the investigation, it is likely that the following led to the malfunction: a likely cause could not be provided.A device history record review revealed no issues that could have caused or contributed to the reported issue.Should additional relevant information become available, a supplemental report will be submitted.Olympus will continue to monitor field performance for this device.
