Event or Problem Description
On (b)(6) 2025, during an endovascular procedure for treatment of an endoleak associated with a previously implanted medtronic graft, a gore® excluder® contralateral leg component was advanced within the patient.Post deployment of the device when removing the catheter through the sheath, the catheter broke approximately 7-8cm down. There was no compromise to the patient. The broken catheter portion and the olive tip were snared and removed from the patient without issue. It was reported there had been some tortuosity, and the physician had experienced some resistance when removing the device. The cause for the breakage is not known however overinflation of the bladder of the dryseal sheath; or too much force during removal (causing the catheter to "snap") may have contributed to the breakage.
