H3, h6: the reported device was received for evaluation.A visual inspection of the returned device found that it is not in its original packaging. The actuator shaft was fractured off the handle and was returned separately. There is biological debris on the returned items.A review of device records showed there were no indications to suggest that the product did not meet manufacturing specification upon release for distribution.A complaint history review found similar reported events.A risk management review found that the reported failure and/or harm was documented appropriately, and there were no indications to suggest the anticipated risk is not adequate. The root cause was associated with a component failure.Factors that could have contributed to the reported event include excessive force applied, where the instrument may have been subjected to higher-than-expected force during activation, leading to structural failure; material fatigue increasing susceptibility to breakage over time; and improper handling or technique, where deviation from the recommended surgical technique, such as improper angulation or excessive torque, may have contributed to the failure.Based on this investigation, the need for corrective action is not indicated.
