This follow-up report is being submitted to relay additional information. The following sections have been updated: b4; b5; d9; g3; h2; h3; h6; h10; h11.Visual examination of the returned product identified the complete femoral construct returned with the anchor plug fractured, not the spindle.Dimensional analysis of the product determined that the product, where measured, was conforming to print specifications. The device was submitted for further analysis.Analysis determined significant post fracture smearing/contact damage is visible, as well as a bend in the anchor plug shaft.Further analysis was able to identify small pockets of fracture artifacts on the surface.Ductile dimples, indicative of overload fracture, were observed as well as possible fatigue striations.Due to the significant post-fracture damage, the failure mode cannot be determined with certainty, however observed artifacts suggest a possible fatigue fracture.Device history record was reviewed and no discrepancies related to the reported event were found.Radiographs were provided and reviewed by a health care professional.Review of the available records identified the following: distal left femur with fracture of the compression spindle of a long stem femoral component with distal femoral resection.Root cause was unable to be determined.Complaint sample was evaluated and the reported event was confirmed. If any further information is found which would change or alter any conclusions or information, a supplemental report will be filed accordingly.Zimmer biomet will continue to monitor for trends.
