One opened 25 ga soft tip cannula in a blister with 2 trocar cannula hub assemblies, and a 25+20k probe was also in the bag was received for the report of difficulty putting the soft tip cannula into port. The returned sample was visually inspected and was found to be non-conforming as there was dried white foreign material on the cannula and the soft tip was cut.The sample was then dimensionally inspected for the cannula outer diameter and was found to be conforming. The sample was not functionally tested for fitting into a trocar as the soft tip was cut by the surgeon.A review of the device history record traceable to the reported lot number, indicates that the reported product was processed and released according to the reported product¿s acceptance criteria. The returned sample was found to be visually non-conforming and dimensionally conforming. The visual condition of the soft tip being cut could result in the reported issue of difficulty putting the soft tip cannula into port.Since the soft tip was cut by the surgeon, a root cause cannot be determined. The cannula met specifications dimensionally. The exact root cause for fit issue with the trocar is unknown, therefore, specific action with regards to this complaint cannot be taken. All assemblies are 100% inspected by trained operators.Complaints are reviewed and monitored at regular intervals for any significant adverse trends. No adverse trends have been observed associated with the reported product and event. No additional action is required at this time. The manufacturer internal reference number is: (b)(4).
