H3, h6: the device was not returned for evaluation; therefore, a device analysis could not be performed. The clinical/medical investigation concluded that, based on the limited information provided, the clinical root cause of the broken meta-nail cannot be confirmed. The x-ray confirms the meta-nail was broken at the distal locking screws, while the fluoroscopy shows an intact meta-nail, possibly from the initial implantation. The patient impact beyond the reported revision surgery remains undetermined.Device batch number was not provided; thus, an evaluation of the manufacturing records could not be performed.A review of complaint history for the previous 12 months did not reveal similar events for the listed device.A review of the instructions for use documents for the intramedullary nail system revealed that possible adverse effects include cracking or fracture of the implant. The preoperative planning section states that the proper type and size of the implant must be selected to ensure effective treatment, considering factors such as the patient's size, strength, skeletal characteristics, skeletal health, and general health.Overweight, musculoskeletal deficient, or unhealthy patients may create greater loads on implants, potentially leading to breakage or other failures.Additionally, the postoperative care section warns that early weight bearing substantially increases implant loading and the risk of breaking the device. Therefore, factors that could contribute to the reported event include the size selected, surgical technique, postoperative care, patient anatomy, abnormal loading of the limb, excessive forces, and/or injury.A review of the risk management file revealed this failure mode was previously identified. The anticipated risk level is still adequate.A historical review concluded that there are no prior actions related to this product and event.According to the inspection drawing, the final inspection includes checking that the part matches the print specifications.Additionally, per the material specification, the quality and manufacturing of ultra-high-molecular-weight polyethylene rod and plate must be controlled. At this time, we have no evidence to conclude that the product failed to meet any specifications at the time of manufacture.Based on this investigation, the need for corrective action is not indicated.Should the device or additional information be received, the complaint will be reopened. No further investigation is warranted for this complaint; however, we will continue to monitor for future complaints and investigate as necessary. We consider this investigation closed.
