Event or Problem Description
It was reported that the¿generator's cord caught fire.¿the issue was found during a therapeutic transurethral resection (tur) procedure that was completed with an olympus back-up device.There was a procedural delay during sedation for less than 30 minutes, but there were no reports of patient harm.

This supplemental report is being submitted to provide the results of the final investigation.Updated fields: g3, g6, h2, h4, h6 and h11.The device was not returned to olympus for inspection and the reported failure was not confirmed.Based on the results of the investigation, and since the device was not returned for evaluation, the definitive root cause of the reported issue could not be determined.Should additional relevant information become available, a supplemental report will be submitted.Olympus will continue to monitor field performance for this device.
