Event or Problem Description
The patient was undergoing a coil embolization procedure in the internal iliac artery (iia) using pod coils, pod packing coils (packing coil), a guiding catheter, a non-penumbra catheter, and a non-penumbra microcatheter. It should be noted that the patient¿s anatomy was mildly tortuous and mildly calcified.During the procedure, while inserting the first pod coil into the microcatheter, the physician experienced resistance while advancing the pod coil at the mid-shaft of the microcatheter and was unable to advance the coil further. After advancing and retracting the pod coil within the microcatheter several times, the physician observed under fluoroscopy that the pod coil had unintentionally detached within the microcatheter. Therefore, the microcatheter containing the detached pod coil was removed from the patient, and the pod coil was flushed out of the microcatheter. The procedure was completed using pod coils, packing coils and the same microcatheter. There was no report of an adverse effect to the patient.

Additional Manufacturer Narrative
Evaluation of the returned pod coil confirmed that the embolization coil was unintentionally detached.Evaluation revealed that the distal tip of the pull wire was advanced distal to the distal detachment tip of the pusher assembly. If the pod coil is manipulated against resistance during use, the pull wire may advance distal to the pusher assembly distal detachment tip and cause the embolization coil to detach. The reported mildly tortuous and mildly calcified anatomy may have contributed to the resistance experienced during procedure.Further evaluation revealed kinks on the pusher assembly and offset coil winds on the embolization coil. This damage is incidental to the complaint. The kink may have occurred during packaging of the device for return to penumbra. The offset coil winds may have occurred during manipulation of the pod coil against resistance.Penumbra coils are visually inspected during in-process inspection and during quality inspection after manufacturing. The manufacturing records for this lot were reviewed and did not reveal any outstanding discrepancies, design, or quality concerns.
