Product complaint #:(b)(4).Investigation summary = > according to the information received, "it was reported that the insert trial broke during left total knee." the product was not returned to medtech orthopaedics; however, photos were provided for review.See attachment (b)(4) device photo ad 18 feb 2025.¿ the photo investigation revealed that attune fb ps trl sz 6 5mm (254505277) had broken. The observed condition of the device was consistent with a component failure that may have been caused by exposure to unintended forces. As the device was not returned, an as-received condition could not be assessed, and a dimensional inspection and document/specification review were not completed. The overall complaint was confirmed as the observed condition of the attune fb ps trl sz 6 5mm would contribute to the complained device issue.Based on the investigation findings, potential cause can be attributed to unintended use error caused or contributed to event and it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the complaint condition. As part of medtech orthopaedics quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot = > the product investigation found no evidence suspecting an error in the manufacturing or material that would be a contributing factor in the reported allegation(s).A manufacturing records evaluation (mre) was not performed.Added: d9.

Product complaint # (b)(4).Investigation summary it was reported that the insert trial broke during left total knee. All pieces were retrieved. There was no delay or extended surgical time. The product was returned to j&j medtech orthopaedics for evaluation.Visual inspection revealed one portion broken from the attune fb ps trl sz 6 5mm's bottom surface.Broken fragment was not returned for evaluation.Additionally, deep scratches were found on the overall surface, as well as heavy usage patterns on it.Breakage observed was consistent with a component failure that was caused by exposure to unintended forces like usage of excessive force in a prying motion while trialing.Furthermore, for the scratched observed, damage can be consistent with other tools and hard edges coming in contact with the device while trialing, or by assembling the device in a misaligned position.Properly handling and¿attention to the approved use of the device diminishes the risk of failure.A dimensional inspection was unable to be performed due to the post-manufacturing damage.A functional test was unable to be performed due to the post-manufacturing damage. The overall complaint was confirmed as the observed condition of the attune fb ps trl sz 6 5mm would have contributed to the complained device issue.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of j&j medtech orthopaedics quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot the product investigation found no evidence suspecting an error in the manufacturing or material that would be a contributing factor in the reported allegation(s).A manufacturing records evaluation (mre) was not performed.
