It was reported that the procedure was performed to treat a lesion in the common iliac artery with a 5mm diameter. After pre-dilatation was performed using three unknown balloon catheters sized at 3mm, 4mm and 5mm, a 5.0x120mm supera self-expanding stent (ses) was advanced to the lesion, over a.018" guidewire and through a 7f sheath, and attempted to be deployed; however, during deployment it was noted that the stent was unable to be fully released. The supera stent was noted to be deployed slightly in the introducer sheath and was removed under fluoroscopy, and replaced with a non-abbott device and the procedure was completed. There were no adverse patient effects nor clinical delay. No additional information was provided.

The device was returned for analysis and the stent was identified to be separated. The reported activation failure was unable to be replicated in a testing environment due to the condition of the returned device.Production record and corrective and preventive actions (capa) reviews were performed and revealed no indication of a product quality issue.Additionally, a review of the complaint handling database identified no other similar incidents from this lot.As there was no damage noted to the device during the inspection prior to use, the investigation determined the reported difficulties appear to be related to circumstances of the procedure as it is likely that during stent deployment the sheath was too close and the stent inadvertently deployed into the sheath resulting in the reported activation failure.Manipulation of the compromised device resulted in the noted kinked inner member. After withdrawal, manipulation of the stent during removal from the sheath resulted in the noted stent stretched/bent/smashed braids and ultimately resulted in the noted stent separation.Based on the reported information and results of the complaint investigation there is no indication of a product quality issue with respect to the design, manufacture, or labeling of the device.B5 - describe event or problem updated.
