Event or Problem Description
The pulsar-18 t3 peripheral self-expandable stent system was selected for treatment of a mildly calcified lesion in a mildly tortuous part of the sfa.The stent could not be released. The wheel on the release device turned without stopping and did not release the stent as intended. The system was removed, and another stent was implanted.

Additional Manufacturer Narrative
The returned product was subjected to a detailed technical analysis and the corresponding production documentation was reviewed to establish whether a deviation from the manufacturing process could be the cause for the reported event. The technical investigation confirmed that the outer shaft has not been retracted.Consequently, the stent has not been released.During the investigation, the device was dissected which revealed that the gluing between the retractable outer shaft and the metal tube which is welded onto the pull-wire has failed. The most probable root cause for the reported event is related to the manufacturing process of a component produced by a supplier. To prevent recurrence the respective internal departments were informed about this case. The supplier of the affected component was informed and acknowledged the complaint.
