This follow-up report is being submitted to relay additional and/or corrected information. The following sections were corrected: h6 the revision was likely due to shoulder dislocation secondary to a fall.A secondary finding is damage to the inferior lip of the liner, likely due to unintended contact with the scapular bone, but it does not appear to have been a contributing factor to the revision. However, the dislocation and any other contributing factors cannot be confirmed as radiographs were not provided and the devices were not available for evaluation. If any further information is obtained that would change or alter any information provided, a supplemental report will be filed accordingly.
