Event or Problem Description

It was reported that during a procedure involving one onyx frontier coronary drug-eluting stent, stent deformation occurred in vivo during positioning/advancement.A non-medtronic wire was used and the old stent was pre-dilated.There was lots of calcium noted on the distal edge of the old stent. It was believed that the onyx frontier stent just hit the struts of the old stent which deformed the stent. The lesion being treated was a severely tortuous, severely calcified lesion in the proximal circumflex artery (cx). The device was inspected with no issues noted.Negative prep was performed with no issues noted. The lesion was pre-dilated.The device did pass through a previously-deployed stent.Resistance was encountered when advancing the device. The procedure was completed using another 2.0 x 30mm onyx frontier stent. No patient complications have been reported as a result of this event.

Product analysis: the device was returned for analysis. The stent was positioned on the balloon between the marker bands as per position specification, but due to stent deformation the stent did not meet visual acceptance specification.A kink was evident to the proximal stent.Deformation was evident to the distal stent wraps with struts raised. No deformation was evident to the distal tip.The inner lumen patency was verified with a mandrel. No other deformation evident to the remainder of the device.Medtronic submits this report to comply with fda regulations 21 cfr parts 4 and 803.Medtronic has made reasonable efforts to provide as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.Medtronic will submit a supplemental report if additional relevant information becomes known.
