Event or Problem Description

According to the reporter, during placement of a high-flow catheter in the right jugular vein, an event related to the metal guide occurred. When passing the guide through the needle, it was not possible to advance or withdraw it, and when removing the needle and the catheter from the vessel, a frayed metal guide was seen that did not come out of the needle (it could not be removed from the needle). An ultrasound scan of the compromised vessel was subsequently performed, and the presence of a hematoma was evident. There was nothing unusual observed on the device prior to use. There was no excessive force used on the device. The guidewire did not break into several pieces. The guidewire from the kit was reported. The toolkit was not used to remove the guidewire. It was removed from the patient along with the needle because it did not exit the needle. The catheter was not repaired. There was no leak. The tego was not utilized. There was no luer adapter issue.Besides the reported issue, there were no other visible defects/damages found on the product at the time of the event. As a remedial action, the clinician chose to remove the guidewire with the needle and order a new catheter kit in which no drawback was obtained; it went uneventfully. There was no medical or surgical intervention needed to prevent a permanent impairment of a function. The event did not lead to or extend patient hospitalization.
