It was reported the scope presented while trying to remove the stopcock the piece disconnected from the scope instead.There were no reports of patient involvement.

The device was returned to olympus for inspection.A root cause could not be identified.Based on the results of the investigation, it is likely the following led to the malfunction: the user request could not be confirmed due to no product return.There were outbound communication attempts documented without response.Should additional relevant information become available, a supplemental report will be submitted.Olympus will continue to monitor field performance for this device.
