It was reported that a patient underwent an atrial fibrillation (afib) procedure with a soundstar and a knotted catheter issue was observed. The soundstar catheter was not able to be manipulated correctly and when trying to pull the catheter out, the catheter had a loop and was not coming out of the sheath. They decided to use fluoroscopy and after gaging visualization of the catheter, it was knotted. The physician pushed the catheter in and out of the sheath and managed to release the knot. The catheter was pulled and then replaced. The problem was resolved. The procedure continued. There was no patient consequence reported.Additional information was received. In the physician¿s opinion, the catheter¿s issue did not put the patient at risk for a potential harm or injury. There was resistance trying to remove the catheter, which told us it was knotted.Once the catheter was visualized under fluoroscopy and straightened out, there was no resistance removing the catheter from the sheath. No detachment of any component. The loop/tip became unknotted. The issue did not result in exposure of any internal catheter components or sharp edges.Fluoroscopy images were not saved, so no images are available.

It was reported that a patient underwent an atrial fibrillation (afib) procedure with a soundstar. There was no patient consequence reported. The device evaluation was completed on 27-mar-2025.The device was returned to johnson & johnson medtech (j&j) for evaluation.A visual inspection and deflection evaluation of the returned device was performed following j&j procedures.Visual analysis revealed several kinks and bents along the shaft.A deflection test was performed and curve p and r were not deflecting totally, but not knotted was identified during the test.Due to this condition, the handle of the device was dissected, and the control wire for each direction was observed properly assembled. There was no evidence for stuck moving which could be affected on the articulation. The damage observed on the shaft, could be related to the manipulation of the device during the procedure; however, this cannot be conclusively determined.A device history review was performed for the finished device batch number, and no internal actions were identified. The kinks and bents along the shaft, identified during the analysis, could be related to the knotted issue reported by the customer; therefore, the complaint was confirmed. The potential cause of the damage cannot be established. The instruction for use (ifu) contains the following warnings: rotate the steering knobs. The steering function should be smooth. The catheter tip should flex in a corresponding direction. If the soundstar® catheter tip does not return to the neutral position after you release the steering knobs, ensure that the tension control knob is completely released.Release the tension by rotating the tension control knob completely in a counterclockwise direction.Position the steering knobs in the neutral position by aligning the marks on the steering knobs to the marks on the housing. As part of johnson & johnson medtech's quality process, all devices are manufactured, inspected, and released to approved specifications.H4.Device manufacture date has been added.During an internal review on 17-apr-2025, noted a correction to the 3500a initial under d4.Primary udi number as should have been left blank. If additional information is received regarding this event, a supplemental 3500a report will be submitted to the fda.Manufacturer¿s reference number: (b)(4).
