On april 30, 2025, the mentor analysis lab received the suspect medical device for evaluation. On may 05, 2025, mentor received the following additional information: the healthcare professional reported that the initially reported device information was incorrect. The identity of the suspect medical device was provided as the following size: 350cc.Brand name: mentor memorygel breast implant.Catalog: 3543507.Lot: 5788434.Serial: (b)(6).Additionally, a yellow-colored right breast seroma was discovered during the revision surgery. The fluid was sent for cytology. There were no reported procedural delays or patient harms due to the findings. On may 06, 2025, mentor completed an evaluation on the returned device.Mentor conducted a visual inspection of the device.Visual analysis of the returned device revealed that the breast implant had a tear extending from the anterior to the posterior view, measuring approximately 6.0 cm.Additionally, siltex cracking was observed on the edges of the tear on the posterior view. The evaluation determined that the possible cause of the rupture is consistent with normal wear.Siltex cracking is defined as a material separation occurring in the siltex layer and can eventually progress through the shell of siltex devices.Siltex cracking is ultimately a result of high stresses.A seroma is a build-up of fluid around the implant.Symptoms from a seroma may include swelling, pain, and bruising.A seroma may occur soon after surgery, or any time later on, which would be referred to as a late seroma. While the body may absorb seromas, some may require surgery for drainage. The american society of plastic surgeons recommends and encourages member surgeons to always submit breast implants, capsule, and effusion to pathology for examination.A manufacturing record evaluation (mre) was performed, and no anomalies were found related to this complaint. In addition, the mre verifies that the device was manufactured in accordance with documented specification and procedures.Manufacturer¿s reference number: (b)(4).
