Event or Problem Description
|
|
It was reported to boston scientific corporation that an encore 26 inflation device was used during an endoscopy procedure performed on an unknown date.During the procedure, while using the inflation device, the manometer's needle did not move, providing no information about the pressure in the dilation balloon. This led to an opening in the esophagus, which was subsequently treated with a clip.It was also reported that withdrawing the device was difficult due to incomplete deflation.Boston scientific has been unable to obtain additional information regarding the patient outcome to date, despite good faith efforts.
