Product complaint # (b)(4).H3, h6 investigation summary: the photo was returned to j&j medtech orthopaedics for evaluation. The j&j medtech orthopaedics team conducted a visual inspection of the returned photo.Visual analysis of the photo revealed that lcp drill sleeve 3.5 f/drill bits ø2.8 had the thread tip broken, the fragment appears to be stuck in one hole of the mating plate.Fracture surface could not be evaluate, due to the image quality is poor to determine any potential cause.Broken condition is consistent with a random component failure that may have been caused by exposure to unintended forces. As the device was not returned, an as-received condition could not be assessed, and a dimensional inspection and document/specification review were not completed. As part of j&j medtech orthopaedics quality process, all devices are manufactured, inspected, and released to approved specifications. The overall complaint was confirmed for lcp drill sleeve 3.5 f/drill bits ø2.8.There is no indication that a design or manufacturing issue has caused the complaint condition.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history review (dhr): part: 323.027 lot: 2232982 manufacturing site: bettlach release to warehouse date: 09 january 2007 a manufacturing record evaluation was performed for the finished article lot and no non-conformances were identified.

Event or Problem Description
It was reported that the patient underwent an unknown surgery for supracondylar fracture of the femur with type c.The devices were used to assist in fixation of medial condyle. They were installed at anterior, and the surgeon tried to bend the edge of the devices with a plier by attaching the sleeve. However, the sleeve tip broke off and came off the plate. It was difficult to remove the broken fragment from the plate, so the other sleeve and one size longer plate were used for the procedure. The surgery was completed successfully within 30 minutes delay.
