Additional Manufacturer Narrative
(b)(4).Date sent 4/23/2025.D4 batch #976c61.Corrected data d4: udi#.Investigation summary- the product was returned for evaluation.Visual inspection and functional testing were conducted on the returned device. The analysis results found that the ecs25b device arrived with no apparent damage with the anvil and washer missing. There were only three partially formed staples present in the pockets and also, dents were observed on the safety. It appears that an attempt was made to fire the device with the safety engaged. It should be notice that to fire the device, draw the red safety back toward the adjusting knob until it seats into the body of the device. If the safety cannot be released, the device is not in the safe firing range. Also, if the firing sequence is not fully completed (the firing handle reaches its stopping point, and the firing trigger is parallel to the instrument handle) staples could be partially deployed. Please reference the instruction for use for more information. The device was reloaded with staples and tested with a test anvil and washer for functionality; the device formed all the staples, as well as completely cut the test media and the breakaway washer without incident. The staple line was complete, and the staples meet the staple form release criteria. The event reported was confirmed and it is related to improper use of the device. Please reference the instructions for use for additional information. It should be noted that as part of our quality process, each device is visually inspected and functionally tested during manufacturing to ensure the device meets the required specifications prior to shipment.A manufacturing record evaluation was performed for the finished device batch number 976c61, and no non-conformances related to the reported complaint condition were identified.
