Event or Problem Description
It was reported that during a small bowel resection the packaging tore in 2 locations when the rn opened the device.Procedure was completed without any surgical delay.No patient consequences was reported.

(b)(4).Date sent: 4/10/2025 photo summary this is an analysis of one photo for evaluation.During the visual analysis, the following was observed: the photo shows a tyvek and it can be seen ripped as if it were delaminated.No conclusion or root cause could be determined.Hands on device analysis may provide the additional evidence necessary to confirm the root cause of the reported event.Because the instrument was not returned our evaluation is limited.A manufacturing record evaluation was performed for the finished device lot number 357d92, and no non-conformances were identified.
