Event or Problem Description

It was reported that, after primary tka surgery had been performed in 2010, the patient experienced knee pain. The x-rays confirmed an issue with the patient's knee. The surgeon discovered that the oxonium surface on the lateral side of the femur, had worn off due to metal-on-metal wear. The poly insert was very worn on the lateral side, confirming the metal-on-metal issue. This adverse event was solved by tka revision surgery on (b)(6)2025, in which the knee was washed out, and one (1) gns ii cmt tib size 6 right, plus the other components, were revised. The patient left the revision surgery in good health. No further complications were reported.

H3, h6: the device was not returned for evaluation; therefore, a device analysis could not be performed. The clinical/medical investigation concluded that the revision was due to pain likely secondary to instability/loosening after 15 years in-vivo with metallic articulation laterally leading to subsequent joint/tissue debris. However, based on the immediate pre-revision and post-revision imaging alone, a definitive clinical root cause of the radiolucency and metallic articulation cannot be concluded and 3rd body wear and/or trauma cannot be ruled out as potential contributing factors.Based on the imaging, a delay in seeking medical attention is suspected.A review of the production order did not reveal a manufacturing abnormality that could have caused or contributed to the reported incident.A review of complaint history for the part number over the past 12 months and for the batch number based on historical data of the device did not reveal similar events for the listed device.A review of the instructions for use documents for knee systems revealed in possible adverse effects section that, although rare, metal sensitivity or allergic reactions in patients following joint replacement have been reported.Implantation of foreign material in tissues can result in histological reactions involving macrophages and fibroblasts. In addition, states that wear of the polyethylene articulating surfaces of knee replacement components has been reported following total knee replacement.Higher rates of wear may be initiated by particles of cement, metal, or other debris which can cause abrasion of the articulating surfaces.Higher rates of wear may shorten the useful life of the prosthesis, and lead to early revision surgery to replace the worn prosthetic components.A review of the risk management file revealed this failure mode was previously identified. The anticipated risk level is still adequate.A historical review concluded that there are no prior actions related to this product and event. At this time, we have no evidence to conclude that the product failed to meet any specifications at the time of manufacture.Factors that could contribute to the reported event include friction or joint tightness.Based on this investigation, the need for corrective action is not indicated.Should the device or additional information be received, the complaint will be reopened. No further investigation is warranted for this complaint; however, we will continue to monitor for future complaints and investigate as necessary. We consider this investigation closed.Corrected data: h6 (health effect - clinical code).
