As reported, when opened the sealed edge of the 3dmax light mesh was noted to be splitting.Review of a photo provided shows a 3dmax light mesh laying in an open clamshell tray. There is a crack/tear present in the seal edge of the mesh and continues into the mesh causing the mesh to separate from the seal edge.Photo review does not assist in determining root cause. As reported the sample is being returned for evaluation; however, has not been received. At this time, no conclusions can be made.If/when the sample is returned and evaluated a supplemental mdr will be submitted.Review of manufacturing records was performed and shows the product was manufactured to specification. To date, this is the only reported complaint for this manufacturing lot of (b)(4) units.Note: section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.
