H11: manufacturing review: the device history records have been reviewed, and this lot met all release criteria.Investigation summary: one 9.5fr power hickman d/l catheter was returned for evaluation.Visual, microscopic, tactile and functional evaluations were performed on the returned device. An in-house syringe with dye water was attached to the red luer. The luer was patent to infusion as water successfully exited the distal end.Aspiration was attempted and was unsuccessful as only air was returned within the attached in-house syringe. Therefore, the investigation is confirmed for the reported suction issues. However, it is inconclusive for the reported reflux within device and difficult to flush as the provided evidence could not replicate the reported failure. The definitive root cause could not be determined based upon available information.Labeling review: as the reported event did not allege a labeling or use related issue, a labeling review is not required.D4 (unique identifier (udi) #), g3, h4, h6 (method, result, conclusion).Section a through f ¿ the information provided by bd represents all the known information at this time.Despite good faith efforts to obtain additional information, the complainant/reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.
