Event or Problem Description
The patient was initially treated for an abdominal aortic aneurysm (aaa) with implant of an afx2 bifurcated stent graft and an afx vela suprarenal aortic extension on (b)(6) 2020.On an unknown date in 2022, a type 1b endoleak coming from the right iliac artery was detected. The physician elected to implant an afx iliac extension in the right iliac on (b)(6) 2022.Additionally, embolization of a lumbar artery due to a type 2 (non-device related) endoleak was performed on an unknown date in 2024.This event was previously reported under mdr number 3011063223-2025-00049.Approximately four (4) years post initial procedure, at the last computed tomography (ct) scan in (b)(6) 2025, the aneurysm diameter measured 103mm, while in 2024 the diameter it was 102mm.The physician suspected there is a type 1b endoleak from the left iliac and elected to treat the patient. On (b)(6) 2025 two (2) afx iliac limbs were implanted. It was reported that it was difficult to pass the lunderquist (non-endologix) wire through the iliac limb; therefore, cannulation was done through the brachial branch to go through the prothesis and it was successful. The two afx limbs were implanted and at the end of the procedure no endoleak was seen. The physician will perform a ct scan in one month.

Additional Manufacturer Narrative
The reported adverse event/incident was investigated in alignment with endologix operating procedures and work instructions. An evaluation of the manufacturing record was completed.A review of the part number/lot number combination needed for device identification shows the device to have been properly manufactured and released in accordance with the device master record. The review confirms there were no manufacturing or processing non-conformities identified that would contribute to the reported adverse event/incident.An evaluation of the device could not be completed. The device was not returned to endologix for evaluation because it remains implanted.A clinical evaluation of the adverse event/incident was completed. An examination of medical records and/or medical imaging received by endologix shows that the aneurysm enlargement and type 1b endoleak of the left common iliac artery are unconfirmed. The additional endovascular procedure is confirmed. This is moderately consistent with the reported adverse event/incident.Device, user, procedure or anatomy relatedness of complaint could not be determined.Procedure related harms could not be determined. The clinical assessment determined a type ii endoleak (non-device related) occurred that was not in the event as reported. The type ii endoleak was discovered during a review of the ct scan labeled post op.Additionally, a 25 mm suprarenal cuff was used in conjunction with a 25 mm main body afx bifurcated stent graft.According to the afx instructions for use (ifu), when selecting a 22mm, 25mm, or 28-mm proximal extension, the diameter should be one size larger than the main body to ensure adequate overlap and sealing. It is unlikely this contributed to the reported event. The final patient status was not reported. No additional investigation of this reported adverse event/incident is planned. However, should additional information relevant to the investigation outcome become available, a follow-up report will be submitted.Endologix will continue to monitor this and similar adverse events/incidents.Corrections: g3: awareness date has been updated.H6: medical device problem codes: remove code 4065.H6: investigation finding codes: remove code 3233.H6: investigation conclusion codes: remove code 11.
