Correction: please refer to section d9 the device was returned and h6 (method, results and conclusion codes). The reported event could be confirmed during the investigation. The device inspection revealed the following: the identification of the returned target device could be confirmed based on the catalog # and lot # marked. The target device was returned assembled with the nail. The devices could be disassembled with the instruments required, however, the disassembling was much harder than usual. Thus, the reported event could be confirmed. After disassembling, it could be observed that the cannulation of the distal part of the target device is covered in dry blood. The blood resulting from the nail insertion could be confirmed as the root cause of the reported event, as it made the instruments disassembling much harder. It must be noted that no major damage was observed on the device.Based on the investigation, the root cause was attributed to blood accumulated around the nail holding screw during the surgery. There were no indications of device or of user related issues.A review of the device history for the reported lot did not indicate any abnormalities. No corrective actions are required at this time.A review of the labeling did not indicate any abnormalities. No indications of material, manufacturing or design related problems were found during the investigation. If more information is provided, the case will be reassessed.
